Neste terceiro ensaio, enfocaremos a visão hermenêutica sobre alegoria e suas relações com a tipologia, destacando a visão dos Padres das Escrituras Sagradas, além da leitura de Santo Agostinho, do Venerável Beda e de São Tomás de (...) Aquino acerca da alegoria hermenêutica. Analisaremos, ainda, as diferenças estabelecidas por Walter Benjamin entre a alegoria e o símbolo, para mostrar como José Saramago reconstrói a alegoria hermenêutica aplicada às Sagradas Escrituras, adotando o ponto de vista benjaminiano, em O evangelho segundo Jesus Cristo. Analisaremos ainda as diferenças estabelecidas pela Retórica clássica entre tota allegoria, permixta apertis allegoria e mala affectatio, considerando ainda como o Maneirismo e o Barroco, inverteram esses valores e de que modo o Memorial do Convento e o Manual de Pintura e Caligrafia, de José Saramago, resgatam a concepção barroca da alegoria construtiva ou retórica. Primeiro de uma série de três artigos que visam a analisar a utilização do discurso alegórico – em sua vertente alegórica e hermenêutica – em três obras de José Saramago: Memorial do convento, Manual de pintura e caligrafia e O evangelho segundo Jesus Cristo. Neste primeiro ensaio, analisaremos os sentidos socioculturais das diferentes avaliações dos processos de leitura e interpretação ao longo da história, tendo em vista sua influência direta na constituição de dois diferentes tipos de alegorias: a retórica e (...) a hermenêutica. Para isso, voltaremos às primeiras práticas de leitura e interpretação, segundo a visão da antiguidade clássica, que subsidiam a limitação dos processos interpretativos que constituem as bases da alegoria retórica, chegando à experiência medieval, com a eclosão da multiplicidade de sentidos e a valorização dos processos interpretativos, que propiciam o surgimento de uma nova alegoria, de natureza hermenêutica. In this essay, and in his honor, I focus on two of physician-philosopher H. Tristram Engelhardt, Jr.’s many contributions, namely, his view that biomedical ethics cannot offer a singular content-driven theoretical approach and requires an appreciation of epistemologies of knowing in medicine. While these two positions remain controversial, because we all want definitive answers to our questions concerning what we ought to do in medicine and elsewhere, Dr. Engelhardt’s view makes possible discussion and debate in medicine to include diverse, defensible (...) ways of knowing. In the end, Dr. Engelhardt’s approach in biomedical ethics is one of shared decision making and negotiation. This is an important model if we take respect for patients seriously in the clinical setting. (shrink)

How can we critique political theory when all we have to use are its own conceptual tools? As Hannah Arendt observed, it can only be done through leaps, inversions, and the turning of concepts upside-down. But this twisting operation must be done in order to turn those who philosophize back to the hard work of real life change. In Turning Operations, renowned theorist Mary G. Dietz challenges specific contemporary modes of theorizing politics-from feminist theory to Habermasian discourse- -while appropriating some (...) of political theory's own approaches and some of its most striking figures, including Aristotle, Nietzsche, Weber, Beauvoir, and Arendt in order to foment some leaps, inversions, reversals and turns on politics along the way. Dietz confronts a number of current debates, arguing that most are filled with artificial division and empty terms. She argues that we must abandon commonly supported dichotomies-masculine versus feminine, speech versus action, liberty versus community -to create abetter discourse, and a better world. Turning Operations is an essential new contribution to democratic and feminist political thought. (shrink)

Background: Healthcare providers who are accountable for patient care safety and quality but who are not empowered to actualize them experience moral distress. Interventions to mitigate moral distress in the healthcare organization are needed. Objective: To evaluate the effect on moral distress and clinician empowerment of an established, health-system-wide intervention, Moral Distress Consultation. Methods: A quasi-experimental, mixed methods study using pre/post surveys, structured interviews, and evaluation of consult themes was used. Consults were requested by staff when moral distress was present. (...) The purpose of consultation is to identify the causes of moral distress, barriers to action, and strategies to improve the situation. Intervention participants were those who attended a moral distress consult. Control participants were staff surveyed prior to the consult. Interviews were conducted after the consult with willing participants and unit managers. Moral distress was measured using the Moral Distress Thermometer. Empowerment was measured using the Global Empowerment Scale. Results: Twenty-one consults were conducted. Analysis included 116 intervention and 30 control surveys, and 11 interviews. A small but significant decrease was found among intervention participants, especially intensive care staff. Empowerment was unchanged. Interview themes support the consult service as an effective mode for open discussion of difficult circumstances and an important aspect of a healthy work environment. Conclusions: Moral distress consultation is an organization-wide mechanism for addressing moral distress. Consultation does not resolve moral distress but helps staff identify strategies to improve the situation. Further studies including follow up may elucidate consultation effectiveness. (shrink)

The social and cognitive science replication crisis is partly due to the limitations of commonly used statistical tools. Inferential statistics require that unsystematic measurement variation is independent of system history, and weak relative to systematic or causal sources of variation. However, contemporary systems research underscores the dynamic, adaptive nature of social, cognitive, and behavioral systems. Variation in human activity includes the influences of intrinsic dynamics intertwined with changing contextual circumstances. Conventional inferential techniques presume milder forms of variability, such as unsystematic (...) measurement error, as in a Gaussian distribution. Inferential statistics indicate an elementary Newtonian cause-effect metaphor for change that is inconsistent with known principles of change in complex systems. Pattern formation in self-organizing systems and quantum probability are used to illustrate theoretical metaphors that instantiate alternative notions of change in complex systems. Inferential statistics and related techniques are crucial scientific resources. However, in the social and behavioral sciences, they must be practiced in conjunction with an appropriate general systems framework that accommodates intrinsic fluctuations and contextual adaptation. On the first page of this very timely paper the author quotes Linda Gordon: This statement provides a theme for response to Jing-Bao Nie's arguments. In reading this paper, I found myself reminded of two of George Orwell's insights: (1) When governments use euphemisms they are usually up to no good: [e.g., the use of for abortion]. (2) Sexuality and the sexual act (I would add here reproduction—having children) can be a powerful tool of subversion and rebellion. One's sexuality (and (...) reproductive ability) can be the last line of defense against repression or authority. (shrink)

Prior studies have shown a general preference among citizens for juries over judges. Researchers, however, have not considered whether race and ethnicity modify this preference. We hypothesized that minorities (African-Americans, Hispanics), who generally express less trust in the legal system, may also express less trust in juries than non-Hispanic whites. We asked a representative sample of 1,465 residents of Texas to state whether they would prefer a jury or a judge to be the decision maker in four hypothetical circumstances. Consistent (...) with expectations, non-Hispanic whites favored juries over judges, particularly if they imagined themselves as a defendant in a criminal trial. By comparison, although African-Americans and some Hispanics generally favored juries, they showed a much weaker set of jury preferences. African Americans had markedly lower support for the civil jury, but support was higher among minorities with prior jury service. Among Hispanics, respondents who took the survey in Spanish typically preferred a judge to make legal decisions. We consider the implications of our findings for trust in the jury system and trust in community members as decision makers. (shrink)

It demonstrates that, by engaging multiple philosophies in concert, the value of aesthetic noise is amplified, thus allowing the listener to better appreciate noise and its possibilities. Providing greater insights into noise as an aesthetic material, Aesthetic Noise will be of interest to researchers and students of sound studies, philosophy, and sound art, as well as sound designers, artists, musicians, and composers. Navigating to goal locations in a known environment (wayfinding) can be accomplished by different strategies, notably by taking habitual, well-learned routes (response strategy) or by inferring novel paths, such as shortcuts, from spatial knowledge of the environment's layout (place strategy). Human and animal neuroscience studies reveal that these strategies reflect different brain systems, with response strategies relying more on activation of the striatum and place strategies associated with activation of the hippocampus. In addition to individual differences in strategy, recent behavioral (...) studies show sex differences such that men use place strategies more than women, and age differences such that older adults use more response strategies than younger adults. This paper takes a comprehensive multilevel approach to understanding these differences, characterizing wayfinding as a complex information processing task. This analysis reveals factors that affect navigation strategy, including availability of the relevant type of environmental knowledge, momentary access to this knowledge, trade-offs between physical and mental effort in different navigation contexts, and risk taking. We consider how strategies are influenced by the computational demands of a navigation task and by factors that affect the neural circuits underlying navigation. We also discuss limitations of laboratory studies to date and outline priorities for future research, including relating wayfinding strategies to independent measures of spatial knowledge, and studying wayfinding strategies in naturalistic environments. Prostate cancer has become a major health concern of male Americans. It is now the most common nondermatologic cancer and the second leading cause of cancer death among men. The incidence of detected prostate cancer rose rapidly in recent years, partly because of prostate-specific antigen testing; it is only now tapering off. Screening for prostate cancer with PSA is widespread in the United States, yet controversial: the American Urological Association recommends PSA screening and the American Cancer Society recommends offering screening; (...) however, the United States Preventive Services Task Force and the American College of Physicians recommend against routine screening; and the American Academy of Family Physicians believes that the decision to screen should be left to the patient. Meaningful participant engagement has been identified as a key contributor to the success of efforts to share data via a “Medical Information Commons”. We present findings from expert stakeholder interviews aimed at understanding barriers to engagement and the appropriate role of MIC participants. Although most interviewees supported engagement, they distinguished between individual versus collective forms. They also noted challenges including representation and perceived inefficiency, prompting reflection on political aspects of engagement and efficiency concerns.

This volume explores, from a variety of perspectives, the political theory of the man who is arguably the greatest English political thinker. It is the first substantial collection of new, critical essays on Thomas Hobbes by leading scholars in over a decade. Hobbes’s writings stirred debate in his own lifetime, for two centuries thereafter, and continue to do so in ours. They emerged in a period of intense political turmoil—a time of civil war and regicide, of puritanical rule and royal (...) restoration. “They were motivated,” Dietz argues, “by concrete political problems and a practical concern, namely, to secure political order, absolute sovereignty, and civil peace.” The contributors emphasize and answer a series of expressly political questions that, to date, have not been fully addressed in the Hobbes literature. They contend that Hobbes’s writings are not mere static artifacts of a particular historical milieu, but rather rich sources of a variety of interpretations and criticisms that spur discussion and debate in their turn. Drawing on a landscape analysis of existing data-sharing initiatives, in-depth interviews with expert stakeholders, and public deliberations with community advisory panels across the U.S., we describe features of the evolving medical information commons. We identify participant-centricity and trustworthiness as the most important features of an MIC and discuss the implications for those seeking to create a sustainable, useful, and widely available collection of linked resources for research and other purposes.

BackgroundThe proper and ethical inclusion of PWLHIV and their young children in research is paramount to ensure valid evidence is generated to optimize treatment and care. Little empirical data exists to inform ethical considerations deemed most critical to these populations. Our study aimed to systematically review the empiric literature regarding ethical considerations for research participation of PWLHIV and their young children.MethodsWe conducted this systematic review in partnership with a medical librarian. A search strategy was designed and performed within the following (...) electronic databases: Ovid MEDLINE, Embase and CINAHL. We screened titles and abstracts using the following inclusion criteria: (1) a study population of PWLHIV or children under 5 years of age; and (2) collection of qualitative or quantitative data regarding ethics of research participation. Excluded were reviews, commentaries, policy statements, clinical care-related ethics concerns, abstracts, case studies, or studies unrelated to HIV research. Studies were appraised for quality, data were extracted, and studies were qualitatively analyzed using a principle-based ethical framework within the Belmont Report.ResultsOf the 7470 titles identified, 538 full-text articles were reviewed for eligibility and only three articles met full criteria for inclusion within this review. While we allowed for inclusion of studies involving young children born to mothers with HIV, only articles focused on PWLHIV were identified. Within the results of these studies, four themes emerged: (1) adequacy of informed consent; (2) consideration of paternal involvement; (3) balancing risks; and (4) access to research and treatment. A strength of this review is that it included perspectives of international research investigators, community leaders, and male partners. However, only two studies collected empiric data from PWLHIV regarding their experiences participating in researchConclusionResearchers and funding agencies should be aware of these considerations and appreciate the value of and critical need for formative research to ensure clinical trials involving PWLHIV promote ethical, well-informed research participation and, ultimately, improve care outcomes. More research is needed to create a comprehensive ethical framework for researchers when conducting studies with PWLHIV. (shrink)

Advances in technologies and biomedical informatics have expanded capacity to generate and share biomedical data. With a lens on genomic data, we present a typology characterizing the data-sharing landscape in biomedical research to advance understanding of the key stakeholders and existing data-sharing practices. The typology highlights the diversity of data-sharing efforts and facilitators and reveals how novel data-sharing efforts are challenging existing norms regarding the role of individuals whom the data describe.
